If you do have to pay, this is NON-REFUNDABLE. Once insurance standing & payment status has been established, then you we will bill your insurance provider directly and you will only be responsible for your co-pay. Pre-determination is attempted before your first visit - but if your insurance rejects, you must contact them to help us resolve the problem. Failure to do so will make you 100% liable for the full charge.
